Greetings! These materials are provided to you at no charge by
Happiness Habit, Inc. in exchange for your honest comments and
feedback. We hope you find them interesting and valuable. Please
read and sign this Agreement BEFORE you begin reviewing them.
1. I agree to tell Happiness Habit, Inc. how I think the attached
materials can be improved and enhanced for my working environment
by commenting honestly and completely on the back of this form.
2. I will not disclose, distribute or duplicate the contents in any way.
I will NOT copy them by electronic or physical means, transfer them
to computers, servers, electronic media or transmit them electronically.
These materials may change dramatically.
3. I am free to share these materials with others and they are free to
review them so long as they agree to abide by ALL these provisions,
sign this Non-Disclosure Agreement and comment as well.
4. I agree to review these materials and comment quickly, ideally
within 48 hours of receipt and return them to: Happiness Habit, Inc.
